P-CARDIAC 風險評估模型由香港大學跨學科的研究團隊開發，是目前首個專門針對華人的心腦血管疾病風險評估工具。 作為P-CARDIAC項目實施階段的重要部分，香港大學研究團隊開創一項HEARTWISE的研究計劃，旨在改善香港公共醫療系統中的心血腦管護理服務。目前HEARTWISE正開展一個為期兩年的研究項目，在六間公立醫院，利用P-CARDIAC協助醫護人員為約3,000名病人度身訂造最合適的健康管理方案。
